The museum itself is mostly handicapped accessible, and arrangements can be made for the Wm. B. Tennison Bugeye cruise is advance notice is given.

The lighthouse is not handicapped accessible either inside or getting to it.

While the underpinnings have been modified somewhat to be safer for visitors, I don't think the interior has been modified at all. You can't pass on the stairs.

Originally I think the exterior would have had a much less safe ladder probably without handrails, and one would have accessed the platform under the lighthouse from a boat instead of stairs with a railing.
