In accordance to some international conventions, Croatia is obliged to provide health care for tourists under the same conditions as to the local population, but there are different kinds of conventions.
The citizens of Austria, Belgium, France (for people with Croatian citizenship), Italy, Luxembourg, Macedonia, the Netherlands, Germany, Romania and Slovenia are entitled to health protection only in case of emergency, namely protection which relieves pain and elements threats to life and death. All other services they have to cover themselves.
Only for the citizens of Germany and Italy there is an exception: they are entitled to have the costs for additional medical treatment covered by their respective countries, but only when they have a valid health insurance in their own country and hold appropriate documents to prove so. If not, they'll also have to pay themselves.
The citizens of the other countries mentioned above are entitled to health protection after presenting the document by their insurance agency (forms HR/A3, JU-6, IT-7, BY-11), which on arrival to Croatia is replaced by a health card issued by the nearest office of the Croatian Institute for Health Insurance. With this card they are entitled to health treatment.
Citizens of Slovenia just have to present their health card.
The citizens of Bulgaria, the Czech Republic, Hungary, Poland and the United Kingdom are entitled to medical assistance on presenting their passports.
The citizens of those countries, with which incomplete conventions (which do not include regulations on health insurance) have been signed, would have to pay everything themselves. They are: Slovakia, Denmark, France, Sweden, Switzerland
