Unless you are dining with a large group of people, the gratuity is not added onto your bill. 15% - 20% is a standard tip. I'm terrible at math, so in CA where tax is 7.5% - I just double the tax.
Don't be afraid to leave a larger tip if the service is excellent - or - no tip if it's terrible. I've been known to tip the busboy as I'm leaving & leave nothing for the waitress who couldn't manage to refill my water even once!
If you're going back more than once - ie: morning coffee across from your hotel - leave a bit larger tip and your service will be exceptional the length of your stay (they'll remember your name too).

**There are many restaurants where you serve yourself & a waitress brings you drinks, clean plates, rolls, and clears your dirty dishes away. These waitresses (with very few exceptions like CA) do NOT make minimum wage. They rely on tips for their income. So please, remember to tip them too.
