I'll talk about the train/metro in Prague a bit later when I have time but as for Airport transportation, take the AAA Radio Taxi cab at the airport to your hotel and back.

At the airport, the AAA Radio Taxi is across the street from the terminal. There is a yellow sign with a couple of employees standing underneath.

Cost to, say a hotel in Prague 2, should be between 500 - 600 CZK. Back as well. Going back, have your hotel arrange a ride with AAA Radio Taxi. This is easier as far as translation/communication goes as well as not being ripped off by other unscrupulous cab companies.
