The only place I know that allows smoking and has walk out balconies is the Sahara. It is definitely one of the 'older' casinos and showing its age. But my cigar smoking friends loved sitting out on the strip view balcony enjoying a good cigar. Not all the rooms there have the balconies, you need to ask ahead.
