Ok, so if i don't tell you how much work it is to get there you'll be cursing me the whole climb... so be warned... but it's worth it 100times over.
First of all get there as early in the day as possible and bring water!!!!!
Ok so go to Prague Castle, and when you buy the titcket to get in the activities, you must buy the one for the Tower in the Cathedral. (It's the best value to get them all).
then, once inside St. Vitus Cathedral, fight your way to the far wall and get in line.
You'll climb hundeds of tightly spiraling steps and just before you feel like you can't go on you'll try to go down but it'll be so packed that you can't, so you keep going.
Once on top it's all up to you. Hang out up there - you deserve it after that climb.
Relax until your muscles stop shaking (there's some benches) and then go crazy with your camera.
I found that a fish-eye lens is most useful and gives the best desired results from the Tower as well as the rest of the Castle grounds.
