Yes, Italy does have a royal family - the Savoia family. They were exiled from Italy after the proclamation of the 1st Republic in which the country voted against the monarchy. The male heirs were forbidden from entering Italy. Until now. The foto you see is at the pantheon on the day of the return of the Italian royals to Rome. The people you see all decked out are super-monarchists who volunteer for this duty. SOme of them stand as an honor guard at the tombs of the Italian kings and queens inside the pantheon.

Often times the Metro pepole will have a control area set up at the bottom of the escalators. You will get a lecture or a fine. If you try and act like a stupid tourist, they will surely give you a fine. If you come clean often times they will let you go as the people in Rome are much less uptight than in Florence, where they can't wait to give a fine.
